The Cleveland School-Based Health Center is
sponsored by the Multnomah Country Health Department and Portland
Public Schools.
Services
- routine physical exams
- early detection, diagnosis, and treatment
of illness and injury
- sports physicals
- mental health services
- age-appropriate reproductive health, including
abstinence counseling,
birth control, STD tests, pregnancy tests, and emergency
contraception
- immunizations
- routine lab tests
- health education, counseling, and wellness
promotion
- fitness and nutrition education and counseling
- prescription medications
- referrals for healthcare services not provided
at the clinics
- mental health services
- vision, dental, and blood pressure screenings
The Health Center is located in Room 163. All
services are confidential. Students are
not charged for services received at the Health Center, but
Medicaid and other third-party insurers will be billed.

- Monday: 7:30 a.m. to 4:00 p.m.
- Tuesday: 7:30 a.m. to 4:00 p.m.
- Wednesday: 7:30 a.m. to 4:00 p.m.
- Thursday: 9:30 a.m. to 6:00 p.m. (note later
times)
- Friday: 7:30 a.m. to 4:00 p.m.
